Dementia support
The dementia guide (from the Alzheimers Society) is for anyone who has recently been told they have dementia. It will help you understand more about dementia and the treatments, support and services that are available. It includes information about how you can live as well as possible with dementia and about making plans for the future. It also contains helpful information for anyone taking on a caring role.

Diabetes information
Solihull CCG is currently working with GP practices together with Public Health Services, Heart of England NHS Trust Diabetes Services, Diabetes UK, and patient representatives. The aim is to:
Improve patient access to diabetes education in order to assist patients to take more control and be supported to self-manage their diabetes.

Diabetes X-PERT Patient Programme. A 6 week programme (each weekly session lasting 2½ hours) for patients on diet only, oral anti-diabetic agents and/or insulin. Run at a variety of times and venues across Solihull for people with diabetes who want to increase their knowledge and confidence to manage their diabetes and medication. Patient can phone the service on 0121 770 4432 to self refer or ask the GP for a referral.
