WE CAN HELP YOU WITH THE FOLLOWING:
Being more active
Financial support

WHAT IS SOCIAL PRESCRIBING?
Social prescribing can help you to have more control over your health and find ways to improve how you feel in a way that suits you .
Social prescribing may be able to introduce you to a community group, a new activity, a local club, or they may help you find legal advice or debt counselling. They could even support you in creating something
new such as a gardening club, a fishing group, a 'men's shed' or a knit and knitter group.
What patients say about us:
"My mother is so pleased as this will make a big difference to her life and getting more help she needs. Many thanks again for all your help and advice, much appreciated."
"Me and my family are now in a much better place and feel more confident and reassured for the future. "
"I want to thank you for all the help you have given me, you've really been supportive and I appreciate everything you have done. "
