Once completed it needs to be returned to the surgery so that the practice nurse can assess which vaccinations (if any) are necessary. Please allow at least 3 working days for the nurse to have time to look at the forms before contacting the surgery for a response. It is recommended that you read the ‘Travel Advice’ leaflet and 'Patient Information - Travel Vaccines' which are also available to download from the website.
