Repeat Prescriptions
If you require repeat medication on a regular basis we will usually issue you with a computerised repeat prescription request form.
You request repeat medication by completing the form.
You may order your repeat medication via the internet using a secure medical record service - 'emis access'.
Telephone requests will not normally be accepted for safety reasons.

Prescription Request Forms
- Tick item(s) required
- Mark 'YES' if prescription should be sent to Windridge Chemist*.
- Deliver, post or fax (01564 771224) to surgery.
- Enclose an SAE if you require your prescription to be returned by post.
- Allow 2 working days before collecting your prescription from surgery.
- Allow 3 working days before collecting your dispensed medication from Windridge Chemist*.

Windridge Chemist, Knowle collect repeat prescriptions daily from the surgery.
If you have marked 'YES' on your form, you can collect your medication directly from the pharmacy.
You may also leave your completed repeat request form at Windridge Chemist, Knowle for delivery to the surgery; please then allow a full week before collecting your medication from Windridge.
Online Prescription Requests Introduction
- Patients may order repeat prescriptions directly from their home via the internet
- Repeat prescriptions are ordered through the secure medical record service - 'emis access'.
- Before using this service, patients need to pre-register at the surgery and then create a secure account.
- Arden Medical Centre only gives patient access to this system for the purpose of requesting their own repeat prescriptions.

Please note ....
Certain medication cannot be listed on repeat prescription request forms, and special arrangements are required to request repeat prescriptions for these drugs
Additionally, the pharmacist that dispenses this medication may ask to see additional documentation e.g.the patient's anticoagulation record book is needed to be produced before warfarin is dispensed.
Patients will be advised of the special arrangements.
