If you are planning to travel abroad and need advice as to which injections are necessary, everyone in your party who is registered with Arden Medical Centre will need to complete a copy of the practice’s 'Foreign travel risk assesment ’ form.

Once completed it needs to be returned to the surgery so that the practice nurse can assess which vaccinations (if any) are necessary.

Please allow at least 3 working days for the nurse to have time to look at the forms before contacting the surgery for a response.
